This form is to be completed if lodging a planning application, request or submission for matters that are to be determined under Part 3, Part 3A or Part 4 of the Environmental Planning and Assessment Act 1979. The requirements for disclosure of political donations or gifts do not apply to applications for a complying development certificate (or modification of such application) or applications or requests made by a public authority on its own behalf or applications or requests that are excluded by regulations. A Disclosure statement of a reportable political donation or gift must accompany a planning application or submission if the reportable donation or gift is made within 2 years before the application or submission is made. If the donation or gift is made after the lodgement of the application, a disclosure statement must be sent to the relevant consent or approval authority within 7 days after the donation or gift is made.

10.4 Disclosure of political donations and gifts
(12) Disclosures of reportable political donations and gifts under this section are to be made available to the public on, or in accordance with arrangements notified on;
(a) a website maintained by the Department (in the case of planning applications or submissions made to the Minister or the Planning Secretary), or
(b) a website maintained by the council (in the case of planning applications or submissions made to that council).
The disclosures are to be made so available within 14 days after the disclosures are made under this section.
